If you are not able to open the above PDF's please call the parks and recreation department at 215-643-3131 or e-mail them at parks@horsham.org and they will send you out a recreation flyer through the mail. If you e-mail please remember to leave your full home address.

Program Registration Form

The above registration form is for recreation programs run by the Horsham Township Parks and Recreation Department. You can now download the form at home and either deliver the form to the Township Building or mail the form in.

Hand Delivered:

This is to help the process move along quicker while you are at the Township Building. We will write a receipt for you and you can be on your way. Payment methods at the township building are cash, check or credit (Do not accept American Express).

Mailed:

If you mail the form in you may only pay with a check. Programs will fill up first come first served so if you mail your registration in, the class may be full. In this case we will VOID your check and call you to let you know that the class is full. If you would like a receipt you must enclose a self-addressed stamped envelope and we will mail you your receipt. There is a $25.00 charge for any returned checks.
